REQUIREMENTS
• 3 copies College Diploma
• 3 copies College Transcript of Record
• 3 copies Birth Certificate/Marriage Certificate
• 4 pcs. 1x1 ID picture
Enrollment Procedure:
1. Secure and fill out the enrollment forms.
2. Submit all the necessary requirements.
3. Submit the properly accomplished forms to the enrolling
adviser in exchange of a registration certificate.
4. Pay the admission fee.
